Immedicare, a partnership between Airedale NHS Foundation Trust and Involve Visual Collaboration, has celebrated its 10th anniversary by expanding the services it provides to care home residents.
The digital service allows care home residents to receive clinical consultations at home, without the need to travel to hospital. Immedicare has recently increased its services to include seven care elements which align with the Enhanced Health in Care Homes (EHCH) framework.
This includes a falls service which enables remote assessments of residents who have fallen, as well as the development of a falls risk assessment and prevention plan. This has resulted in 85.5% of residents being able to remain in their home after a fall, instead of a potentially unnecessary trip to A&E. In addition, Immedicare has a Consultant Pharmacist as part of its multidisciplinary team to support medicines optimisation and offer advice in complex cases.
The service has also launched a series of core skills training programmes, which have been specifically designed to upskill care home staff and provide the best possible outcomes for their residents. Over 1,200 care staff have completed the courses over the past year.
Along with the enhanced services, Immedicare has unveiled a new brand identity in celebration of the 10-year milestone. The new branding will be seen across all of Immedicare’s digital marketing channels, and will also have a presence within the Digital Care Hub, based at Airedale NHS Foundation Trust.
